Death Appointment

Important
- Only book an appointment if the medical examiner has made you aware that the paperwork has been sent to us. If a GP has issued the medical certificate, it must be reviewed by a medical examiner. Until that has happened you cannot book to register the death. Medical Examiners work in the bereavement office at the hospital. They will telephone you regardless of where the death took place. They will then confirm that you are able to book an appointment to register.
- We request that a maximum of two people attend to register a death at a register office.
